NTSB safety-culture fixes: FAA, Boeing The 737 Max door-plug pop-out was caused by sloppy practices that had been reported by inspections for years. Boeing management chose to let the practices continue, since it had not caused them a loss of money. The FAA reported them to Boeing repeatedly but did not compel it to change them. Perhaps the FAA took for granted that an airplane company would prioritize safety, and was not prepared to deal with one that would prefer to cut corners on safety. |
